It is our goal for each client to have an arrangement that will meet their specific needs and to understand our relationship prior to funding. And remember, we are here to answer each and every question.
1. Complete the Application Download Mazon PDF application form 
A. Complete the application and return it with the following:
- Copy of DBA certificate (assumed name certificate from courthouse) or Copy of Incorporation or Partnership Certificate and Articles and Bylaws.
- Copy of each of the principles' driver license.
- Copy of your general liability and workers comp insurance certificate, if your business requires this.
- Financial Statement (profit/loss statements and/or balance sheet, if available).
- Accounts Receivable Aging (list of unpaid invoices).
- Master customer list with name, address with zip code, and telephone.
- One copy of a sample invoice with appropriate backup documentation (purchase order, shipping, etc.).
- Guaranty for each owner of the business.
B. Mazon will review your information and upon approval will prepare documentation (step 2) for both parties.
2. Documentation
- Factoring Agreement and Security Agreement covering the terms of the business relationship.
- Certified Copy of Corporate Resolutions (if incorporated), indicating Board of Directors approval to factor.
- Guaranty, signed by majority owners of a corporation.
3. Initial Funding
Mazon will assist the client in the preparation of paperwork necessary for the initial funding. |
